About your server defacement by Ir4dex : I found notes dating from 2004/01/07 about it without any 'conclusion'. By now, may be you know how they did proceed. If it's not the case, as one of my friends server+site have been defaced too the 2004/01/17 by them, I've studied the Ir4dex methods, and I'm quite sure that they did : 1) deviate a PHP page call to any of your server sites PHP acessible pages. Doing so, they can manage, either to read directly security files ( as /etc/passwd ), either, anyhow, at end create new server PHP malicious files ( for ex. under /tmp ) 2) then they scan server directories, and are even able to gain root privileges access via different means including litteral code execution 3) After site study, they place backdoors, use server sites mailers for spam or indirection, deface all the 'index' files, etc...
